Your Rights and Responsibilities

You have the right to:

  • Respectful treatment and privacy
  • Choice and involvement in your care
  • Access to primary health care services
  • Access to specialty services outside of HCCC
  • Access to your medical records
  • Information in your primary language
  • Understand your treatment options
  • File a grievance without worry of retribution

You have a responsibility to:

  • Schedule appointments when you need medical care
  • Cancel appointments 24 hours prior appointment date / time
  • Request timely medication refills 72-hours before they are due
  • Cooperate with your health care team and play an active part in your care planning (e.g. labs, diagnostic imaging, follow-up care)
  • Treat all HCCC employees, including your provider, with respect and common courtesy
