Hill Country Community Clinic Nurse Practitioner and Physician Assistant Fellowship Program


Preparing Nurse Practitioners and Physician Assistants to provide high-quality, affordable integrated healthcare in rural Northern California.


The Hill Country Community Clinic Nurse Practitioner and Physician Assistant Fellowship program trains fellows to become a valued member of a patient centered integrated health team.  Our program focuses on the medical specialty of caring for the unique and challenging underserved population with a focus on the poor and underserved.  Through a positive learning environment, team based whole patient care, state of the art modern clinics, educational opportunities, and timely constructive evaluations, our fellows become fully competent medical providers who provide high quality care to their patients.

Overview of the Program

Our 24 month long program will be structured with a slowly grown patient panel, weekly didactic education, preceptorship clinic, mentorship clinic, specialty clinic, and a quality improvement project.  The goal is to build on the knowledge obtained in the Nurse Practitioner and Physician Assistant Programs with a focus on a medical home model including general and preventative medicine, behavioral health, addiction medicine, Women’s Health, Family Planning, Chronic Disease, Pediatrics, Psychiatry, Chiropractic, Podiatry, Hepatitis C treatment, and Complex Case Management services.  Hill Country Health and Wellness Center is a true healthcare home for our patients, dedicated to healing the whole person and the fellows are important members of the team.


Applicants must have a Masters in Nurse Practitioner or Physician Assistant studies from an accredited institution. PA candidates need to be NCCPA certified and licensed to practice in the State of California. Similarly, NP candidates should be certified by either the AANP or ANCC licensing board as well as licensed to practice in the State of California. In addition to meeting all entry requirements, applicants will have a strong desire to learn the skills necessary to practice primary care within the community health clinic setting and treat the underserved populations of our community.

The residency is a full-time, 24-month salaried position with benefits.


  • Depending upon experience, $90,000 – $93,600 annually, $45/ per hour

Current Fellowship

Laura Houdek FNP-DNP

Quality healthcare growing up was so hard to obtain in a small rural community setting. I new that if I wanted to make changes in those small rural communities, I needed to follow my dreams and become an NP. My studies took me to Seattle Washington at Seattle University. I received my advanced practice provider as a Public Health Nurse and continue on to receive my family practice NP, followed by my Doctorate. During my years in Seattle, I was a part of Seattle a children’s Abusive Head Trauma team, specifically working with shaken baby syndrome. I work on implementing a prevention program in the state of Washington as well as creating a curriculum for high school males. After I received my Doctorate, I taught for Seattle University College of Nursing in their Public Health track. As much as I loved teaching, I knew I needed to get back into patient care and revisit the reason I went into the medical field. I came across Hill Country’s fellowship program and their mission statement, morals, and philosophy couldn’t run more parallel to mine. It was the idea of wrap around care for each patient that really drew me in. It was always something I thought about, you have this patient in front of you right now and you need to do anything and everything at this one visit because you don’t know when you are going to see them again. Hill Country embodies this. This fellowship has taught me I’m not alone. There are so many people and resources to access, with the main goal is patient centered care. I have loved working with this patient population and utilizing everything we have to offer! When I’m not at work, I love to cook and bake! I cannot wait for lake season and kayaking!

About Hill Country

Hill Country is a small FQHC located in Shasta County, California. Our main site is quite rural, and we also operate satellite sites in Redding, the largest city in far Northern California.
